Requested Documents


We will review your service history and specific questions and concerns prior to meeting with you. In addition, we will take the time necessary to discuss and clearly explain your retirement or survivor benefits to you as well as your spouse or surviving beneficiaries.

  • CSRS or FERS annual Notice of Annuity Adjustment (sent in December to retirees)
  • Copies of beneficiary designations for CSRS, FERS, TSP, FEGLI, Unpaid Compensation (as applicable)
  • Copy of a recent leave and earnings statement (employee only)
  • Recent Retirement Estimate to include all service history (employee only)
  • Most recent Social Security Benefit Statement
  • Most recent Thrift Savings Plan Participant Statement
  • Copies of Death Certificate to include with benefit claims
  • Any communications from the Office of Personnel Management, your employing agency, Social Security Administration, Thrift Savings Plan, etc. relevant to your questions and concerns
